Request a repeat prescription


If you’re an existing patient of Your Local GP, you can use this form to request a repeat prescription. 

Please note the following important information:

• There is a prescribing charge of £20 to generate a prescription, as each request requires a review of your record to make sure it is safe to prescribe, and we are taking responsibility for that prescription.
• There will be additional charges for medication from the pharmacy which will vary according to the treatment prescribed.
• Your doctor may need to see you for a consultation before issuing your prescription, we will confirm this with you when we receive your request. This will be a paid for consultation in addition to the prescription fee.

Please ensure you are as accurate as possible with the name of your medication, dose and any other details that we need to be aware of, so that we can review and issue your prescription promptly. We recommend requesting your prescription 1 week in advance, to ensure you do not run out of treatment.

Once requested, we will review your request and issue a repeat prescription. You can indicate your preferred pharmacy, or you can use our recommended pharmacy partner. If you have any difficulties using the form below, please contact us by email.
